A Case of Vitiligo in an 11-Year-Old

We present here a case of an 11-year-old girl who developed depigmented patches on her right chin 6 months back. She noticed the areas of depigmented skin to steadily increase in size with no clinical symptoms. Two months later, she noticed various erythematous hyperkeratotic lesions on her face and extremities due to a cold, for which she visited the physicians. 

 

She denied any family history of vitiligo and psoriasis. Her laboratory tests and physical examination were normal. Dermatological examination revealed well-defined, irregularly shaped depigmented patches on her chin; and scaly erythematous plaques on various sites, such as the right cheek, the right hand, the ear and lower limbs. Her lesions were positive for the Auspitz sign. 

 

Laser scanning confocal of the psoriasis lesion on her right hand showed hyperkeratosis, incomplete keratinization, hyperplasia of the epidermis, apical dermal papilla, capillary tortuous dilatation, hyperemia and peritubular inflammatory infiltration. 

 

She showed positive Wood′s lamp examination for the depigmented patch on her chin.

 

Thus, she received the clinical diagnosis of co-occurrence of vitiligo and psoriasis; and topical agents like tacrolimus and steroids for her lesions. After 12 weeks, she showed great improvement in her lesions.
